The careful balancing of Medication Care, Physiological Care, Social and Family Care has now created a mental route for the patient. Extending a purposeful life of the cared coupled with an improved social interaction.
Integration of care includes Social Care monitoring and control of the cared consumption and continence is reported to the Medical Support Team for medication control.
The importance now is the up to date document all of the sequence of the daily care, its procedures, timing, medication, physical support and patient wellbeing.
Its background is the availability of information to correctly give to Care at Home.
Maintain an up to date support as caring changes, provide the means of a constant reporting system.
Also to be able to hand over at any time the Care At Home document of caring thereby keep the patients daily programme in sequence.
In conclusion and from experience, it would have been impossible to maintain the life of the cared and to enjoy the support of a family, without the integration of the teams headed by the GP and Medical Support Team.
